Careers in Pharma for Doctors: What Can You Actually Do?

If you're a doctor considering leaving clinical medicine, or simply wondering what else you could do with your medical degree, the pharmaceutical industry can feel surprisingly difficult to understand.


You may have heard of Medical Affairs. Perhaps you've come across job titles such as Medical Advisor, Medical Science Liaison or Medical Director on LinkedIn.

But what do these people actually do? What other roles are available? And, importantly, which pharma jobs specifically need doctors?


The answer is: there are a lot more options than most doctors realise.


Doctors work across the pharmaceutical industry, from designing and medically overseeing clinical trials for experimental medicines to providing medical leadership for products that are already used by patients around the world.


Some roles keep you very close to clinical medicine and science. Others involve strategy, safety, regulation, evidence generation, communication or compliance.


Here are some of the main career paths worth knowing about.


1. Medical Affairs: Medical Advisor to Medical Director


Medical Affairs is one of the most common areas doctors encounter when exploring careers in pharma.


It sits at the intersection of medicine, science and the business, helping ensure that medical and scientific expertise informs how a medicine is understood and used.


Doctors entering Medical Affairs may work in roles such as Medical Advisor or Medical Affairs Physician. With experience, they may progress into positions such as Medical Lead, Medical Director, Therapeutic Area Lead, Country Medical Director or Global Medical Director.


A Medical Advisor might be responsible for a particular medicine, disease area or portfolio.


Their work could include:

  • Interpreting clinical trial and real-world data

  • Developing and delivering medical strategy

  • Working with external medical experts

  • Supporting scientific meetings and congresses

  • Identifying evidence gaps

  • Supporting studies and evidence-generation activities

  • Reviewing scientific and medical materials

  • Training internal colleagues

  • Providing medical input into cross-functional activities

  • Working with regulatory, market access, commercial, safety and clinical-development colleagues


As you become more senior, the role generally becomes increasingly strategic.


A Medical Director or Medical Lead may have responsibility for the medical strategy of an entire product or therapeutic area, lead other Medical Affairs colleagues and make higher-level decisions about evidence generation, scientific engagement and medical activities.


Medical Affairs isn't necessarily country-based either.


Large pharmaceutical companies often have local, regional and global Medical Affairs organisations, creating opportunities to progress between different levels of the business.


Do you need to be a doctor?


Not always.


Some Medical Advisor and Medical Director positions specifically require physicians. Others are open to pharmacists, PhDs and other scientific professionals.


Always read the job specification rather than assuming from the title.


Who might enjoy it?


Someone who enjoys medicine and science but also likes strategy, communication, collaboration and seeing the bigger picture.



2. Medical Science Liaison (MSL)


Medical Science Liaisons are usually field-based members of Medical Affairs teams.


Their role centres around scientific exchange with healthcare professionals and external medical experts.


An MSL might spend their time discussing new clinical data with specialist physicians, attending congresses, understanding changes in clinical practice and bringing insights from the medical community back into the organisation.


It's important to understand that an MSL isn't a pharmaceutical sales representative.


The role is scientific and non-promotional.


Doctors can make excellent MSLs because they're comfortable having peer-to-peer scientific discussions with clinicians. However, MSL roles aren't exclusively physician roles.


Do you need to be a doctor?


Usually no.


MSLs commonly come from medical, pharmacy, PhD and other advanced scientific backgrounds.


Who might enjoy it?


Someone who enjoys meeting people, discussing science, building professional relationships and attending conferences.


And because these roles are often field-based, being comfortable with travel is important.



3. Clinical Development Physician / Medical Monitor


If you enjoy clinical research and evidence-based medicine, Clinical Development is one of the most important areas to explore.


It's also an area where being medically qualified can be particularly valuable.


Two job titles you'll commonly encounter are Clinical Development Physician and Medical Monitor.


What does a Medical Monitor do?


A Medical Monitor provides medical oversight of a clinical trial.


While investigators at individual trial sites are responsible for participants at their site, the Medical Monitor looks across the study from the sponsor's perspective.


Responsibilities can include:

  • Providing medical oversight of an ongoing clinical trial

  • Reviewing adverse events and emerging safety information

  • Answering medical questions from investigators and study teams

  • Reviewing eligibility questions

  • Assessing protocol deviations from a medical perspective

  • Identifying potential medical or safety trends

  • Contributing to protocol development

  • Reviewing clinical data as it accumulates

  • Working with Clinical Operations, Drug Safety, Data Management and Biostatistics

  • Supporting medical decisions about the conduct of the study

  • Contributing to interpretation of trial results


Because Medical Monitoring involves making medical judgements around trial participants and patient safety, these positions almost always require a medical degree, although exact requirements vary between companies and studies.


And don't just look at pharmaceutical companies.


Medical Monitors also work for biotech companies and CROs (Contract Research Organizations).


What does a Clinical Development Physician do?


The titles can overlap, but a Clinical Development Physician may have responsibilities extending beyond the medical monitoring of an individual study.


They may contribute to the overall clinical development strategy for a medicine.


That means helping answer questions such as:

Which patient population should we study?

What should the next trial look like?

Which endpoints are clinically meaningful?

What dose should move forward?

What evidence will regulators require?

Do the emerging efficacy and safety data justify continuing development?


More senior physicians may eventually have medical leadership responsibility for an entire clinical development programme.


When searching for opportunities, try job titles such as:

Medical Monitor

Clinical Development Physician

Clinical Research Physician

Study Physician

Clinical Development Medical Director


And be careful with the title Medical Director.


A Medical Director working in Clinical Development isn't necessarily doing the same job as a Medical Director in Medical Affairs.


Always look at the function and responsibilities rather than the title alone.


Do you need to be a doctor?


For Medical Monitor and physician-designated Clinical Development positions, usually yes.


Who might enjoy it?


Someone who enjoys clinical trials, research, patient safety, evidence-based medicine and making medical decisions from complex data.



4. Early Development, Translational Medicine and Clinical Pharmacology


Some doctors work much closer to the beginning of drug development.


This is where promising scientific discoveries start making the journey from laboratory research towards becoming medicines that can be tested in patients.


Physicians working in Early Development, Experimental Medicine, Translational Medicine or Clinical Pharmacology may help answer questions such as:

How does this medicine behave in humans?

What should the first dose be?

What dose should we take forward?

Which biomarkers should we measure?

Which patients might benefit most?

Are the early biological and clinical signals strong enough to justify further development?


Clinical Pharmacology can involve areas such as pharmacokinetics, pharmacodynamics, dose selection and first-in-human studies.


These positions can be particularly attractive to doctors with strong academic or research interests.


Do you need to be a doctor?


It depends on the position.


These are multidisciplinary fields containing physicians alongside pharmacologists, pharmacists, scientists and other specialists. Positions specifically designated as physician roles will generally require medical qualifications.


Who might enjoy it?


Someone fascinated by mechanisms of disease, pharmacology, experimental medicine and the science behind how new treatments move from laboratory to clinic.



5. Pharmacovigilance / Drug Safety Physician


Medicines don't stop being monitored once a clinical trial ends or a product reaches the market.


Pharmacovigilance (PV), or Drug Safety, is concerned with understanding, evaluating and managing the safety of medicines throughout their lifecycle.


Physicians working in safety may evaluate adverse events, review individual cases, assess potential safety signals and contribute to the ongoing evaluation of a medicine's benefit-risk profile.


Imagine receiving reports of a particular medical event in patients taking a medicine.

Is this something that occurs naturally within this patient population?

Could it plausibly be related to the medicine?

Are similar cases starting to appear?

Does the pattern require further investigation?

Does something need to change?


Answering those questions requires medical judgement alongside expertise from many other safety professionals.


Drug Safety physicians may also contribute to aggregate safety reports, risk-management activities, safety governance and interactions with regulatory authorities.


Do you need to be a doctor?


Not every Pharmacovigilance job requires a doctor.


However, there are specific Drug Safety Physician, Safety Physician and Medical Safety positions for which medical qualifications are typically required.


Who might enjoy it?


Someone analytical and detail-oriented who enjoys medical problem-solving, assessing causality and looking for patterns across complex clinical information.



6. Medical Information


Medical Information is a career path that doesn't always get the attention it deserves.


Medical Information (MI) teams provide accurate, balanced and evidence-based information about medicines.


Healthcare professionals may contact a pharmaceutical company with questions that aren't necessarily answered by the prescribing information.


For example:

Is there evidence for using this medicine in a particular patient population?

Are there data about a particular drug interaction?

What evidence exists around pregnancy or breastfeeding?

Has the medicine been studied alongside another treatment?


These enquiries can require significant scientific investigation.


Medical Information professionals search and critically evaluate clinical trials, publications, guidelines and company data before developing an appropriate response.


They may also develop standard response documents, maintain scientific information resources, review emerging evidence and work closely with Medical Affairs, Pharmacovigilance, Regulatory and other functions.


This is an area where doctors, pharmacists and other clinically and scientifically trained professionals can bring considerable value.


For doctors, your ability to understand the clinical context behind a question can be particularly useful.


Do you need to be a doctor?


Usually no.


Medical Information teams often contain pharmacists, doctors and life-science professionals.


Pharmacists in particular have a strong presence within Medical Information.


Who might enjoy it?


Someone who enjoys researching clinical questions, critically evaluating literature, writing and communicating scientific information accurately.


If you love being asked a difficult clinical question and disappearing down a PubMed rabbit hole to find the answer, Medical Information might be more interesting than you think.



7. Medical Signatory / ABPI Final Signatory


This is a career option doctors exploring the UK pharmaceutical industry should know about.


Under the ABPI Code of Practice, promotional material must be appropriately certified before it is issued.


For certification under the relevant provisions of the Code, the nominated signatory must be a registered medical practitioner or a pharmacist registered in the UK. A UK-registered dentist can act in relation to a product solely for dental use.


That means doctors and pharmacists have access to a responsibility that isn't open to every professional working within pharma, with dentists also eligible in the specific circumstances set out in the Code.


But being a signatory isn't simply proofreading marketing materials.


A signatory needs to consider whether materials and activities comply with the ABPI Code and relevant requirements and whether scientific and medical claims are appropriately supported by evidence.


You might review:

  • Promotional materials

  • Claims based on clinical studies

  • Materials used by field teams

  • Meeting and event materials

  • Digital content

  • Certain medical and educational materials

  • Activities involving healthcare professionals


It's a significant responsibility.


And having a medical degree doesn't automatically make you ready to become a signatory.


Companies need appropriately experienced people with sufficient product knowledge, industry experience and detailed, up-to-date knowledge of the ABPI Code.


Signatory responsibilities may form part of a broader Medical Advisor or Medical Director position. However, there are also dedicated Medical Signatory, Final Signatory and Medical Compliance roles.


The work isn't necessarily confined to a traditional UK affiliate structure either.


People working within UK-headquartered or multinational organisations may have broader regional or global responsibilities, although the specific certification requirements depend on which rules and Code apply to the material or activity.


Do you need to be a doctor?


For ABPI certification, the eligible professions are specifically defined.


Doctors and UK-registered pharmacists can act as nominated signatories, with UK-registered dentists eligible for products solely for dental use.


Who might enjoy it?


Someone meticulous who enjoys critically evaluating evidence and is comfortable taking responsibility for difficult decisions.

It can be particularly interesting if you like the intersection between medicine, science, ethics and compliance.



8. Regulatory Medicine / Regulatory Affairs


Before a pharmaceutical company can market a new medicine, it has to demonstrate its quality, safety and efficacy to regulatory authorities.


That's where Regulatory Affairs comes in.


Regulatory teams develop the strategies and submissions required to obtain and maintain approval for medicines.


Many Regulatory Affairs professionals aren't doctors.


However, physicians can work in Regulatory Medicine and provide medical input into clinical evidence, regulatory strategy, benefit-risk discussions and interactions with health authorities.


And doctors don't only work on the industry side.


Physicians can also work for medicines regulators themselves.


Do you need to be a doctor?


For most Regulatory Affairs positions, no.


Specific Regulatory Physician or Regulatory Medicine positions may require a medical degree.


Who might enjoy it?


Someone who enjoys evidence, strategy and precision and is interested in understanding how decisions about medicines are made at a regulatory level.



9. Real-World Evidence, HEOR and Evidence Generation


Randomised controlled trials are incredibly important.


But they don't answer every question.


Once a medicine starts being used in routine clinical practice, companies, regulators, clinicians, payers and healthcare systems may want to understand what happens in broader patient populations and real-world settings.


That's where Real-World Evidence (RWE) comes in.


Doctors may contribute to identifying clinically meaningful research questions, designing studies, interpreting data and translating findings into evidence that can inform decision-making.


Related areas include Health Economics and Outcomes Research (HEOR) and broader Evidence Generation.


Depending on the organisation, these activities might sit within Medical Affairs, Market Access, Epidemiology, Data Science or dedicated evidence-generation teams.


Do you need to be a doctor?


Usually no.


These are highly multidisciplinary areas involving epidemiologists, health economists, statisticians, data scientists, pharmacists, physicians and other researchers.


But clinical expertise can be extremely valuable.


Who might enjoy it?


Someone interested in research and data who particularly wants to understand what happens outside the controlled environment of a clinical trial.



10. Medical Communications, Publications and Medical Education


Clinical data aren't particularly useful if nobody can understand them.


Pharmaceutical companies generate enormous amounts of scientific information that needs to be communicated accurately to different audiences.


Doctors may work in or alongside teams responsible for:

  • Scientific publications

  • Congress presentations

  • Medical education

  • Scientific communications

  • Internal medical training

  • External educational programmes


Some of these positions sit directly within pharmaceutical companies.


Others exist within specialist medical communications agencies working with pharmaceutical and biotech clients.


Doctors can also work as medical writers, scientific directors and medical education specialists.


Do you need to be a doctor?


Usually no.


Medical communications attracts people from medical, pharmacy, PhD and broader scientific backgrounds.


However, clinical experience can be particularly valuable when translating complex evidence into something relevant to healthcare professionals.


Who might enjoy it?


Someone who loves writing, teaching and turning complicated science into something another human can actually understand.



11. Medical Roles in MedTech and Diagnostics


Don't automatically restrict your search to pharmaceutical companies.


Doctors also work in medical device, diagnostics and health technology companies.


A medical device company developing surgical technology needs people who understand how that technology will actually be used by surgeons and patients.

A diagnostics company developing a new test needs people who understand where that test fits into a clinical pathway.


Depending on the organisation, you may encounter roles such as:

  • Medical Advisor

  • Medical Director

  • Medical Affairs

  • Clinical Affairs

  • Clinical Development

  • Medical Safety

  • Medical Education


If you've decided you want to move into “industry”, don't assume that means your next employer has to manufacture medicines.


Do you need to be a doctor?


It depends enormously on the position and technology.


Some roles specifically require physicians or particular clinical specialties. Others are open to a much wider range of scientific and healthcare backgrounds.



Which Pharma Roles Actually Require a Doctor?


This is an important distinction.


Having a medical degree can be an advantage for many industry roles without being a requirement.


For example, Medical Affairs and MSL teams may contain doctors alongside pharmacists, PhDs and other scientific professionals.


Medical Information similarly employs people from several professional backgrounds.


But there are other positions where companies are specifically recruiting a physician.


These commonly include roles such as:

Medical Monitor

Clinical Development Physician

Clinical Research Physician

Drug Safety / Safety Physician

Certain Medical Director positions


And then there are responsibilities such as ABPI Final Signatory, where the Code itself defines which healthcare professionals are eligible to certify material.

This distinction matters when you're searching for your first industry role.

Don't assume every job containing the word “medical” requires a medical degree.


Equally, don't undersell yourself by searching only for generic pharmaceutical jobs when there are positions specifically looking for the clinical judgement that physicians bring.



What About Pharmaceutical Medicine as a Specialty?


Another thing many doctors don't realise is that Pharmaceutical Medicine is itself a recognised medical specialty in the UK.


There is a formal Pharmaceutical Medicine Specialty Training pathway covering areas including drug development, clinical trials, medicines regulation, drug safety and the medical aspects of commercialisation.


But that doesn't mean you need to complete specialty training in Pharmaceutical Medicine before you can get your first industry job.


Doctors enter the industry through a variety of routes and at different stages of their clinical careers.


Your first task is understanding which part of the industry actually interests you.



So, Which Pharma Career Is Right for You?


Don't start with job titles.


Start with what you actually enjoy doing.


Love talking to clinicians and discussing science? Explore MSL and externally facing Medical Affairs roles.


Enjoy strategy, evidence and working across different teams? Explore Medical Advisor and broader Medical Affairs positions.


Love research and clinical trials? Look at Clinical Development Physician and Medical Monitor roles.

Enjoy analysing difficult clinical cases and identifying patterns? Drug Safety and Pharmacovigilance could be worth exploring.

Meticulous about evidence and comfortable making compliance decisions? Medical Signatory work might suit you.


Love writing and teaching? Explore Publications, Medical Communications and Medical Education.

And remember: you don't have to know exactly where you want to end up before making your first move.


Your first pharmaceutical industry role is exactly that, your first role.


Doctors can move between functions, therapeutic areas, products, countries and companies throughout their industry careers.



How Do You Actually Find These Jobs?


One of the easiest mistakes to make is simply typing “pharmaceutical jobs for doctors” into a job board.


Companies don't necessarily advertise them that way.


Search using the actual industry job titles.


Try:

Medical Advisor

Medical Affairs Physician

Medical Science Liaison

Medical Monitor

Clinical Development Physician

Clinical Research Physician

Study Physician

Clinical Development Medical Director

Drug Safety Physician

Safety Physician

Medical Information

Medical SignatoryFinal Signatory

Regulatory Physician

Medical Director


And don't only search Big Pharma.


Look at large pharmaceutical companies, smaller pharma companies, biotech companies, CROs, medical device companies and diagnostics companies.


You may find that the role that gets you into industry isn't at the company you initially expected.



Your Medical Degree Hasn't Locked You Into One Career


One of the hardest things about considering a move away from clinical practice is that medicine can make your career path feel predetermined.


Medical school.

Foundation training.

Specialty training.

Consultant or GP.


It's a very visible ladder.


Industry doesn't work like that.


The career paths are less obvious — but there are far more of them than many doctors realise.


Your clinical training has given you skills in scientific reasoning, evidence evaluation, communication, decision-making, risk assessment and understanding patients that can be incredibly useful outside direct patient care.


The challenge is learning how those skills translate into the language of industry.

So before deciding that pharmaceutical industry careers aren't for someone like you, spend some time exploring what's actually there.


Search the job titles.


Read the job descriptions — including some you're not currently qualified for.

Look at the backgrounds of people already doing those jobs.


You'll start seeing the same skills, responsibilities and career pathways appearing repeatedly.


And suddenly, “working in pharma” stops looking like one mysterious alternative career.


It starts looking like an entire collection of careers you didn't know existed.
